Soft tissue conditions like Myofasciitis with trigger points characterized by diffuse muscle pain, dull aching pain is worth a special mention in back and neck pain conditions. Although the exact pathophysiology is unknown, many patients might have metabolic changes associated with hormonal imbalances, malnutrition, Vitamin deficiencies like Vitamin D, Vitamin B and Vitamin E, mineral and electrolyte deficiency, stress disorders etc. The condition is often clinically diagnosed with no confirmed investigatory evidence although some are positive in rheumatology tests. Iontophoresis is the introduction of drug locally in the form of ions. the advantage is that the drug reaches the target area in correct concentration and any untoward effect of the drug is minimised.Iontophoresis can even save a surgery. For eg: acetic acid iontophoresis can be used to remove calcium deposits from around a joint. Acetate ions introduced by this way combines with calcium ions to form calcium acetate which is a soluble form and removed by the body. Many cases like Tendonitis, Myositis, Bursitis, neuritis,Neurovascular deficits, arthritis, Sprains and other soft tissue injuries, edema, myositis ossificans responds quickly and effectively to Iontophoresis

Bells palsy : or lower motor neuron lesion is characterised by flaccid paralysis in which the muscles of one side of the face is hypotonic. It is due to lesion of the seventh cranial nerve the facial nerve which supplies the corresponding side of the face. Rarely bilateral presentations are also seen. The nerve may be affected at anywhere along its long course in and around the skull, most common being viral infections, middle ear infections, parotid ( salivary gland) inflammation or infection. The symptoms usually relieve with three weeks of time with intervention in even severe cases. Patients present with varying degree of involvement and no two patients are identical in all the presentations. Physiotherapy in the form of electrotherapy, manual stretching and strapping will augment the recovery and patient is left with little or no deficits.
Peripheral nerve injuries and pathologies: associated with trauma like traction injuries after high velocity trauma like road traffic accidents. Often these are closed injuries with no visible cuts or bruises but the patient presenting with severe neurological pain with tingling sensation, altered or hypersensitivity to cold and hot, numbness, glistening or dryness of skin, swollen fingers or toes with redness and warmth etc. An example is the Brachial Plexus injury. In open clean cut nerve injuries and crush injuries there can be obvious muscle loss or atrophy, complete loss of sensation and movement / paralysis. If physiotherapy is administered at the appropriate time, we can even save the limb or part from going into complete paralysis. Yet another case of peripheral nerve involvement is tumours both benign like a lipoma or carcinogenic one which affects the nerve. Vitamin deficiencies, drug related neuropathies (eg: peripheral neuropathy after chemotherapy), diabetic neuropathy, radiation neuropathy, neuropathy after electric shocks are some cases which we effectively manage. Circulatory neuropathy after trauma or embolism although rare is also among the conditions in our 18 year old clinical database. Nerve injuries after injection ( Injection palsy ) of the sciatic nerve or the radial nerve is also a condition which Physiotherapy is of great help. Physiotherapy is also essential part of the rehabilitation process after microvascular surgeries where severed limb is sutured back by a skillful long surgical process. Post surgical physiotherapy is unavoidable for complete recovery. Sideways curve of the Spine is known as Scoliosis and a mild angulation can be seen in most people. Right and left part of human body is not symmetric in all aspects. Size and orientation of the pelvic bones, angulation of the hip joint, limb length discrepancy of the long bones etc contribute to Pelvis although the exact cause cannot be always ruled out. However the human body adapts to it with mild angulation most commonly in the lower lumbo - pelvic and lumbar area. This is often characterized by a counter balancing curve at thoracic area and may be yet another at the cervical area too. The sideways curvature evokes a muscular imbalance of the left and right sides and the persons with moderate curvature presents with symptoms of back pain, dull aches and uneasiness in early adolescence. In mild angulation, the patient usually have a pain free life when spine is flexible and tend to develop pain in early forties when spinal mobility starts to lessen. Our experience in treating a large number of persons from mild, moderate to severe scoliosis will help you in managing your spine well. Detailed assessment along with an elaborate patient education with a set of therapeutic exercises will help the patient to get self motivate to achieve the goal.
